Person Centered Planning
At RE Health, we strive to provide curated care plans that fit the unique needs of each individual. We use person-centered-planning to evaluate the needs of each individual, set attainable SMART goals, and track overall progress.
Therapy
As an additional level of care and to assist with person centered plan assessment, continued growth, and crisis management, we partner with local therapists to provide both individual and group therapy sessions two times each week.
Daily Living Assistance
We teach and assist with daily living skills to support autonomy and growth for each resident. Things like: personal hygiene, cooking, cleaning, orderliness, laundry, and more.
Education
We partner with local county schools, magnet schools, intermediary, and day treatment programs to provide the necessary educational support for each resident.
